Campbellsville Elementary School Family Resource Center and Campbellsville Middle and High School Youth Services Center personnel are working with Big Smiles to offer dental services for students.
The centers are working with Big Smiles, who will provide state-licensed dentists to come to CES and the CMS/CHS campus to provide checkups, cleanings, x-rays, fluoride treatments and more.
Forms to request an appointment have been sent home with students.
At CES, appointments will be made for Thursday, Feb. 20, from 8 a.m. to 3 p.m.
At the CMS/CHS campus, appointments will be Wednesday, Feb. 19, from 8 a.m. to 3 p.m.
For those who have medical insurance, that insurance will be accepted.
Appointments will also be offered for students who do not have dental insurance and those covered by Medicaid and Kentucky Children’s Health Insurance Program.
Cost for those without insurance will be $83 for those age 13 and younger and $90 for students age 14 and older. Parents can also request donated care to cover the cost.
Those who are covered by Medicaid and/or KCHIP must provide the student’s ID number.
Applications for appointments can be submitted online at www.myschooldentist.com or submitted to the FRC at CES or the YSC at the CMS/CHS campus.
Additional applications are available at CES and the CMS/CHS main offices.
Appointments will be offered in fall and spring.
