CES to host kindergarten registration week May 4-8

Campbellsville Elementary School will host registration the week of May 4 for students entering kindergarten in the 2020-2021 school year.

 

To enroll, children must be 5 years old on or before Aug. 1.

 

Parents should submit registration paperwork, which is accessible online, during the week of May 4.

 

Students need seven items before they can officially enroll at CES. Those items are listed below.

 

Items one, two and three must be submitted now, but CES staff members understand it could take additional time to compete items four, five, six and seven. Those items can be submitted at CES open house this summer or on the first day of school.

 

1. Enrollment packet, which is available by clicking HERE or can be picked up at the CES main office.

 

2. Proof of residence.

 

All custody papers, if applicable, must also be submitted during the application process.

 

3. Certified birth certificate from the Department of Vital Statistics in Frankfort.

 

The birth announcement that is received from the hospital at the time of birth or the billfold-sized copy will not be accepted. A full-sized certificate from Frankfort is required.

 

Birth certificate applications are available at the CES office or the Taylor County Health Department.

 

It takes three to five weeks for an application to be processed. Cost is $10.

 

4. Up-to-date immunization record on a Kentucky certificate. The health department or doctor will update immunizations.

 

5. Preventative health care examination, within six months prior to first enrollment in school.

 

Physicals must be reported on forms prescribed by the Department of Education entitled "School Medical Examination Form."

 

Physicals after Feb. 1 will be valid. It is best to make an appointment with a doctor or health department as early as possible to avoid the August rush.

 

6. Eye examination, within six months prior to first enrollment in school.

 

Eye exams must be reported on forms prescribed by the Department of Education entitled "Kentucky Eye Examination Form for School Entry." Eye exams after Feb. 1 will be valid.

 

7. Kentucky Dental Screening/Examination form.

 

Dental screenings/exams performed after Feb. 1 will be valid.

 

There are several ways to submit enrollment forms.

 

Forms can be mailed to Campbellsville Elementary School, c/o Donna White, 315 Roberts Road, Campbellsville, Ky., 42718.

 

Forms can be faxed to (270) 789-3827.

 

Forms can be scanned and emailed to donna.white@cville.kyschools.us.

 

Or, forms can be brought to the CES office during the week of May 4 from 9 a.m. to 2 p.m. each day.

 

Those who would like to bring their forms to the school should ring the buzzer at the front door and wait for someone to help them.

 

For those whose children currently attend preschool or head start, registration will be completed automatically at those locations.
